As a new practice, Aging Grace House Calls is proud to offer innovative care model for older adults in need of primary care.
Our convenient house calls allow patients to receive care in the comfort of their own home, eliminating the hassle and stress of traditional doctor’s visits.
Our nurse practitioner is highly experienced in geriatric care, providing a comprehensive approach that puts the needs of the patient front and center.
Meet the Nurse Practitioner
Kathryn Mordino ANP-BC
As a Nurse Practitioner with over a decade of experience, I understand the importance of comprehensive healthcare based in preventative care and guideline-directed interventions. I studied nursing at Boston College and graduated with a Masters in Nursing in 2012. Since then I have been developing a practice with a focus in primary care, chronic disease management, substance use treatment and geriatrics care. I moved to Bangor with my husband and three young daughters in 2023. I started Aging Grace House Calls to add a robust care option for adults aging at home in the Bangor area.
OUR SERVICES
LAB TESTS AND SCREENINGS
Regular visits allow you to keep on top of your medical needs and ensure you are always up-to-date when it comes to the important matter of your wellbeing. Labs, screenings and referrals can be provided.
CHRONIC DISEASE MANAGEMENT
It can be difficult to manage chronic illnesses such as diabetes, COPD, heart failure. Appointments with our provider will help to simplify medication regimens and ensure you are receiving the most personalized and effective care plan.
PHYSICAL EXAMS
Making sure you are up to date when it comes to Physical Exams is extremely important. Schedule an appointment with our experienced and caring provider to make sure nothing is overlooked when it comes to your health.
HOUSE CALLS
All visits are conducted in the comfort of your own home. The provider bring all the tools for robust assessment/treatments. Care plans can be tailored to your living environment. Family members can attend visits easily.
PATIENT PORTAL
In compliance with ONC and Cures Act - all patients have access to a secure online portal. Patients can access visit notes, care plans, meds list. We encourage you to use the portal for non urgent communications including appointment scheduling, prescription refills and medical questions.
WHOLE HEALTHCARE
We focus on care of the whole person. Our care plans include primary prevention, chronic disease management, urgent care and transitional care between hospitals and nursing home stays. Care plans will be evidence based and personalized to your health goals.
